In Search of Home-based Service Network for the Old Aged Community Care in France

Abstract
In France, who are recipients of home health services or home assistance programmes receive primary and community care with the conventional and fragmented professionel and organisation of services-that is, general practitioner¡¯s regular ambulatory and, home visits, nursing and social services, home aids, and meals on wheels. Clients falling between primary and secondary health care and social services are risk of being forgotten. A possible solution may be the coordination of medical and social services in a continuum of care with ¡®gerontological network¡¯. This determined the ability to design care plans and coordinate all available agencies, thus assuring intergrated care. A contrario, the joint mobilization of plurality of actors in the field is not evident. This article explores the reasons the actors in the field try to establish ¡®gerontological network¡¯ as well as the difficulty in networking all the professionals concerned.
